Connecting Communities. Expanding Access. Improving Lives.

Our Partners

At United Community Network, we believe that collaboration is key to creating stronger, healthier communities. Our Partners program connects with a wide range of organizations—including hospitals, health systems, government agencies, nonprofits, schools, employers, and faith-based organizations—to maximize the reach, quality, and effectiveness of our services. Together, we create a network of support that empowers individuals and families across all 50 states.

Through strategic partnerships, we ensure that our clients have access to the resources they need to thrive. By combining expertise, sharing resources, and coordinating services, our partners help expand the impact of our programs in healthcare, education, housing, employment, mental health, and social services. These collaborations strengthen our community initiatives and ensure sustainable, long-term results for the populations we serve.

Hospitals & Health Systems

We collaborate with hospitals and health systems to provide seamless access to medical care, specialty services, and community health initiatives. These partnerships enhance patient navigation, preventive care, and chronic disease management while ensuring quality, coordinated healthcare.

Medicaid & Medicare Plans

By working with Medicaid and Medicare plans, we help individuals navigate coverage, maximize benefits, and access essential services. Our collaborations ensure that members receive timely, comprehensive care and support for preventive and chronic healthcare needs.

Government Agencies

Partnerships with local, state, and federal agencies allow us to connect clients with public programs and resources. These collaborations help expand access to benefits, housing, food assistance, transportation, and other critical support services.

Nonprofits & Community Organizations

We work closely with nonprofit organizations and community groups to provide additional services, referrals, and programs. These collaborations amplify our impact by addressing unmet needs, reaching vulnerable populations, and enhancing overall community well-being.

Schools & Universities

Educational partnerships enable us to support early childhood programs, school readiness, tutoring, and higher education initiatives. Collaborating with schools ensures children and young adults have the resources, guidance, and access needed to succeed academically and socially.

Faith-Based Organizations

We partner with faith-based organizations to reach communities, provide culturally competent services, and support initiatives that strengthen families and neighborhoods. These partnerships are essential for community engagement and trust-building.

Employers & Businesses

Working with employers and business organizations allows us to support workforce development, health programs, and economic mobility initiatives. These partnerships create opportunities for employment training, small business support, and community-based workforce programs.

Community Organizations

Local community organizations are critical allies in delivering services directly to neighborhoods. Our collaborations with community groups help address immediate needs such as food, housing, transportation, and emergency support, ensuring equitable access to services.
